QUALIFICATIONS OF HOME HEALTH CARE
1. Patient must be under the care of a Physician
2. Patient’s physician must certify that patient needs one or more of the following services for a medical reason:
- Skilled Nursing Care
- Physical Therapy
- Speech Therapy
- Continued Occupational Therapy
3. Patient must be home bound or must experience a considerable or taxing effort to leave home.
