What is Home Health Care?
Home Health Care is skilled nursing care, therapy, and aide services provided in the comfort of your home for the treatment of an illness or injury.
Am I eligible for Home Health Care?

There are four conditions that must be met in order for you to be eligible for home health services:

  • You must be homebound. This is defined as a normal inability to leave home. Physical conditions and/or limitations, such as to cause a considerable and taxing effort to leave home.
  • Your physician must decide that you need medical care in your home and order it for you.
  • There must be a skilled need, such as intermittent skilled nursing care, physical therapy, speech-language pathology services, or continue to need occupational therapy.
  • The care must be medically necessary.
How do I receive Home Health Care?
Express your interest by discussing Golden Age Health, Inc. with your discharge planner, social worker, or physician. Before care begins, you will receive an initial in-home assessment by a registered nurse or a physical therapist that will help you identify your specific needs, discuss your care plan, and resources available in your community. An individualized plan will be developed and approved by your physician outlining the care to be provided. How quickly can services be provided?
Most patients are seen within 24 hours of referral.
Where are services provided?
Care is provided in the home, as well as assisted living facilities and retirement communities.
When are services available?
Care is available seven days a week. We also have Registered Nurses on call 24 hours a day.
How long do services last?
Services may last a few weeks to several months. Patients are discharged when goals are met or when services are no longer needed.
Can I change the scheduled visit if I have an appointment or other engagements?
Changes can easily be made by calling our office. Visits can be rescheduled for a time that is convenient for you.
Will my insurance cover Golden Age Health, Inc. care services?
Our exceptional home health care services are well within reach for most patients. Medicare, private insurance, Managed Care Plans, Veterans Administration, and Workers’ Compensation typically cover many of our services.
Do you offer help with filing insurance claims?
Yes. Our reimbursement specialists conveniently provide assistance with insurance verification, authorization, reimbursement levels, and other insurance issues.
If I have a question whom do I contact?
