Liaison between patient, caregivers, and community agencies. Assists the physician, family members, the patient, and other persons involved with long range planning.

Patient Education, Disease Teaching, Medication Management, Wound Care, Nutritional Analysis, Pulmonary Care, Cardiovascular Care, Diabetes Management, Respiratory Care, Bowel and Bladder Training, Foley Teaching.
HOME HEALTH AIDE SERVICES
Assist with activities of daily living, bathing, personal care, walking, medication reminders, meal preparation, light housekeeping, laundry and other household services to maintain a safe and sanitary environment as well as services ordered by your physician.
OCCUPATIONAL THERAPY
Activities of daily living, home skills, equipment assessment management and cognitive retraining
PHYSICAL THERAPY
Ambulation, Strength, Balancing Exercises and Mobility Training, Equipment Assessment Training, Anodyne Therapy, Myofascial Release
